The Healthy Living Program information is linked below. Please get your physicals as early as possible to prevent any future issues. Remember that you are entitled to an annual physical based on the calendar year, ie. January through December. The cutoff for the submission is June 30th, 2019, but be advised that if you wait till the last minute to submit your forms you could be asking for a disaster. If your forms are not received in time you will have to provide proof that you submitted them prior to the cutoff.
